Description
Daybreak makes it possible for people with dementia in Oxfordshire to live well and remain at home for longer.
Through providing fun social activities like singing, as well as mental exercises, they slow cognitive decline, reduce hospitalisation, and delay the need for long-term care, easing pressure on a stretched NHS. Referrals to Daybreak double annually, but their fragmented IT system drains staff time and holds them back from meeting the rising tide of need.
The investment from The Fore will transform their operations with a streamlined IT system. This will free up staff time to expand services, unlock new funding streams and develop a national dementia care evaluation tool, paving the way for better dementia care across the country.
